Simple Explanation
The reason for the existence of this devotee who is most anguished [due to being unable to bear the existence in material world] is bhagavAn’s desire only.

One who is greatly anguished due to
- the adikkodhippu (the heat under the feet) as said in thiruviruththam 1 “inninRa nIrmai ini yAm uRAmai” (I cannot tolerate the lowly life here), thiruvAimozhi 3.2.1 “ennAL yAn unnai ini vandhu kUduvan” (When will I come and unite with you), thiruvAimozhi 3.2.9 “enginith thalaippeyvan” (after missing out previously, where will I reach you?), thiruvAimozhi 9.8.4 “nALEl aRiyEn” (I don’t know the day when I will reach you), thiruvAimozhi 5.8.7 “thariyEn ini” (I cannot sustain anymore) and in thiruvAimozhi 6.9.9 “kUvik koLLum kAlam innam kuRugAdhO” (Will the time to invite me to engage in those divine feet, not happen soon, even at this stage?), and
- the great thirst acquired by enjoying bhagavAn.
prArabdha karma (the karma which has started reaping its results) cannot be said as the reason for such anguished devotee, since that is contradictory to what is said in SrI bhagavath gIthA 18.66 “sarva pApEbhyO mOkshayishyAmi” (I will relieve you from all sins). This is so since prapaththi’s (surrender’s) nature is as explained in SrI pAncharAthram “ArthAnAm ASupaladhA sakrudhEva kruthAhyasau | dhrupthAnAmapi janthUnAm dhEhAnthara nivAriNI ||” (Will the surrender which is done once, not give the result soon to one who is anguished and not eliminate next birth for those chEthanas who are satisfied with that?) Hence the reason for the existence of the devotee’s body in this world is ISvaran’s desire of “Let me enjoy him with this body itself for sometime”. This is why piLLai lOkAchAryar is saying “kEvala bhagavadhichchiyiRE” (It is bhagavAn’s exclusive desire). Since there is a factor of bhagavAn’s desire in the dhruptha prapanna (those who are satisfied to get the result at the end of this birth) in addition to prArabdha karma, here by adding “kEvala” (only), that is being differentiated.
